Auto populate "This is Me" and Support Plan from Initial intake assessment
Being mindful of double handling we would like to see a Intake form that covers all the areas within the "This Is Me" and "Support Plan" that auto populates into these tiles. Finding a lot of double handling of information and looking for ways to reduce administration

If the Care Plan would have a review button as the Outcomes have it would be great. Also, it would be good if there was the possibility of seeing past entries on the history mode as you have on the outcomes feature.
